Issue Summary
I have just enabled the pm_block_user.module but the "block sender" link does not appear in user pages (user/uid). I don't see any permission/configuration option to that effect, and I have not defined any blocking rule. Am I missing something? Theme is Garland. I am also using content profile.

got it... I now see that the link shows up in the messages, not in sender's user page, as I expected.
Does that means that the only way to unblock sender is to keep his/her message?

Yes, right now, that is the only way. See #572806: Add list of blocked users
