admin menu bar only appear below page

Having some strange problems and even on fresh install the black bar only appears in vertical column beneath the page. Trying to rebuild my site after some obscure failure and everything is going wrong.
Warning Mac users using Fetch to FTP
Found error. I was uploading using Fetch on my Mac - no problem and then for reason I don't understand yet it was automatically attaching .txt to every file it thought was text including .php files. Caused havoc and took me 15 hours to track down why my site was behaving erratically and nonsensically. A programmers and maintainers nightmare.
Also I discovered I must delete the module folder before uploading newer version it doesn't replace folder with newer version but sort of mingles the results.
Beware - I was ready to cut my wrists since we had a mail drop that was hitting and no web site to respond. Nice!
