Blank feedback page...

So...

I finally "found" my feedback page by searching the forum archives but I can't figure why it's blank. I successfully installed the module, configured the thing, added a new feedback page, set up an alias and there it is...

a page with a title and a "created by" line and...

that's it.

I thought maybe I had to add the fields manually in the module but when I opened it up, it looked like there was already supposed to be stuff in the node. What's up with that?

I don't recall seeing a troubleshooting section for blank feedback pages in the documentation anywhere.
