I tried the patch provided, read the instructions. No block showing up - any other assistance/ideas for fixing this? Also, not using masquerade so that isn't an issue either.

I have the same issue too. I read the usage of readme.txt and applied a patch regarding user_access() function from another issue thread.

I logged in with user id 1 (the first admin user) but the block now shown up.
