jamesbisset commentedI'm having the same problem, but I have a few more clues as to why it's happening in my case.
First off, here's the example:

As you can see, I'm using imagemenu and Fancy Slide seems to be hi-jacking the thumbnails from the Imagemenu erm... menu to display in the control bar. As there are three menu images, only for the fourth slide does the correct thumbnail display. Further, the Alt text displayed also belongs to the Imagemenu images.
Of course, it could be an Imagemenu issue, because if I remove the Imagemenu block, then Fancy Slide works as expected.
PS: I've already overridden the theme_imagemenu_item() function to remove the rollover code - don't know if that's helpful or not, but it obviously doesn't get rid of the problem.

chriscohen commentedThanks for reporting this extra information. It sounds very much like both fancyslide and imagemenu are using the same id attributes in the HTML. This is something we will need to look into in more detail, but hopefully a fix won't be too hard if that's the case!

There's a patch attached to this post which should fix your mismatched thumbnail issues. I've given it a quick test and it seems to solve it.
