Add another item gives error [#646816]

I've got a content type called "video" with the number of items set to "unlimited"

When I click "Add another item" I get the following error and prohibits upload. I can save the node, hit edit, then add another item, however:

I can't see the rest of the error. If I knew what was happening I might be able to figure out what module would be causing the error. If this was just me, I wouldn't really mind the workaround, but my users won't add videos if it's too hard!

I've got the same problem with add another image on another content type. Any ideas?
