I want to set up a large site, starting with a community forum.
- one group of generally accessible forums
- thousands of small closed user groups, each with their own forums, structured similarly or identically
- per closed user group, kind of CMS function with possibility to upload documents
- per closed user group, 2 or 3 levels of users with different rights
- each CUG to be moderated by own moderator, supervising user registration, ... to be appointed by webmaster.

It's more complex than this, but in a nutshell this summarizes it.

Anyone who knows if it can be done with Drupal?
- out of the box?
- out of the box + extra modules?
- out of the box + lots of custom programming?

Hi, have you looked into using Organic Forum as part of Organic Groups module

Organic Groups are for sub-sites. And it's a pain. Check this module http://drupal.org/project/simple_access

definitely sounds like a use case for og-- and no, it's not for subsites (though you can use it that way). For an idea of whats possible see groups.drupal.org.
