fixed on checkbox

Hi,

i found out that the team notification ui did not work when i check on the box. It seems that the checkbox shows all being checked, and there is about 2-3 checkbox at wrong places...
I've done a fix on this..
Please review the changes .
And touch it up to match some padding issue ..
