It doesn't seem to be quite kosher for the original author to get possible commisions based on use of this module, so removing the option of having the original author's associate id used.

That was actually the intent, allowing people to point their commissions to the author or the Drupal association if they wanted to use Amazon module for simple use cases like book review thumbnails and so on, but I don't have any problem pulling those options out. They weren't really used that often and anyone who cared enough to change it from the default was more likely to want to use their own affiliate code.
