When enabled, this module is highlighted as being incompatible with aggressive caching on the performance tab. Is url alter fundamentally incompatible with such caching, or is it possible to make it compatible?

Hmm, I don't think it should be technically incompatible. It's marked that way because we needed to add an empty implementation of hook_boot() so that our code is loaded by the time we get to the path loading bootstrap.
