With this module enabled, hitting preview will not display the comment along with the node body. Only the edit comment page shows up.

Yep, we see this as well.

This is based on #302240: button broken due to fix various problems when using form storage .

The problem is in from caching. Try reseting cache, and hitting preview button.
Any ideas?

No! This is not duplicate of this.

We have the same problem... but we use live to give users some of preview functionality.

D 6.15 Added 'Comment Upload' today. 'Preview comment' is set to optional (for default, required for content-type) but 'save' button is missing now. Exact behaviour as in original thread start post. Both comment upload perms are checked.

With the latest version of Drupal core I no longer see this problem. My instance of it was related to some site-specific code doing a form_alter that wasn't behaving properly.

This seems to be really hard to track down!

Confirming that upgrading to 6.16 fixed my issue.

The image appears in the comment, only if the 'list' box is checked in the file attachment area which I find a little unusual but never-the-less, preview works!
