Add safety guard in luceneapi_index_open() to prevent fatal errors if hook_init() hasn't been run

As a result of the issue #666826: Class 'LuceneAPI_Exception' not found when running update.php, a safety guard should be put in place to prevent fatal errors even in the rarest of instances.
