OK - I've created a form using CCK and it works great. But I was hoping the users would be able to submit the information to the form - so it would tied into the site's theme.

Right now the form is a 'create content' choice and not associated with the site's theme.

Any help much appreciated!

OK - I discovered I can create the form via the Profile Module and it works great.
