I've had the above problem with more than one CCK field modules, specifically in this instance the FileField module.

I'm working with a custom theme, and notice that if I'm using a default theme, everything works fine.

Thoughts? Is there a script I should add to my template file or something?

Created this issue when I was a complete novice to Drupal theme creation.

If I remember correctly, I named all of my improperly named theme files correctly, and then everything worked great!
