I love this and really need this module - thank you!
I am only seeing 4 categories on the "alter" admin page...

History (auto generated)
Contact Information (my custom category)
My Family (my custom category)
Orders (Ubercart)

I would like to turn off History and Orders, which I can do.
But I also have a category "Medical Information" and I cannot see that in the admin page or on the profile page once the module is turned on. It was there before - are we limited to 4 choices?

OK, I see now that I have to have information entered in at least one field in that area in order for the categories to show up. This is logical but not obvious when first making the profile form and trying to arrange the order of the fields.
