I just need Name field instead of "First Name" and "Last Name". Also email address field and "confirm" email address field with checking of both values are same and then submit. What is the best way to customize this? Thanks,

Not sure about the e-mail confirmation, but the Name/First/Last fields in your mailchimp.module subscription form are pulled from the settings for the given list in MailChimp. If you change the fields for this list in MailChimp, the fields on the subscription form on your Drupal site will change to match.
