If a menu item has been defined on the node edit page, and the node is not currentlyu published, the associated menu item should be removed/disabled from the menu tree.

Upon publishing the node, the menu item should be restored.

Not sure when this was posted, but in Drupal 7 (and 6 too), menu links are not displayed when you don't have access to the node behind that link.
