I have had this website running for over 2 years and it attracts quite a few visitors daily so I am sure to moderate all comments made my users myself before they go live

Over the past week or so, a handful of comments in the the forum appeared to have bypassed any sort of moderation queue and are going live straight away. I have to go through each one and delete it. Why is this happening. I have disabled comments for users altogether, but I have still see what changes this brings.

That is very odd indeed. While you're figuring out the cause, you may want to try the Mollom module. Many of us use it to block spam and it has Drupal's founder behind it, so it's a very well thought out module and service. Just a thought, best of luck.
