When I send out test e-mails to multiple e-mail addresses, sometimes some recipients the header and footer that were specified through simplenews template are not included. It appears to be a random issue since some recipients do see the header and footer. Has anybody encountered this issue and/or have any suggestions?

Turns out the e-mails with no header and footer were somehow using the wrong newsletter template (I had more than one defined). I found a bug report at http://drupal.org/node/682566.
