Hi. I just upgraded to the latest version of the form, and for some reason it no longer shows the contact form to authenticated or anon users, only the admin user. I saw the note: "Note that the block now only appears for authors when they have their 'personal contact form' enabled." I enabled the personal contact form and it still doesn't bring back the forms. Also, the block permissions are set to allow anon/authenticated users to be able to see the form.
Any help greatly appreciated!

the.alphy commentedNevermind. It turns out that selecting the content type for the block doesn't work if you don't also specify the urls in the "Page specific visibility settings."
Seems like you shouldn't have to specify the urls if you tell it which content type..

mallton commentedI had to specify 'Anonymous' and 'Authenticated' roles in order for the form to display for all visitors, and then had to specify which URLs on which to display the form.

The block will only show when i set "Show block only on specific site" and then i set "content/nodename

royerd commentedWhen the obvious fixes fail, double check your install to make sure there is not a random "contact" folder up in the root with nothing in it. This would cause the form to fail.
