Can you say what sort of work will be required to migrate private_upload settings to D7?

Would it be something that could be accomplished with a SQL query? Or would files need to be moved, permissions changed, .htaccess changed, etc?

It will not be ported to Drupal 7. Drupal 7 handles private and public files natively.

Drupal allows for separate private and public fields but not private and public in one.
So this module could potentially provide that addition.

This module could well bridge that gap, so please reconsider its future :-)
Many sites which use public files, would benefit from having private files method available for example based on roles/content types/sections, etc.

If not, can anyone provide any guidance on developing the key element of this module, a Public/Private toggle for attachments, in Drupal 7?
