With PHP Version 5.2.12 and Apache 2.
It was working on my previous server without any error. But when I migrated to another one, I receive it even I tried it again on ANOTHER new server. So, 2 new servers with the same error.
The funny part is, when creating a node with upload files, the file is uploaded successfully, and I can even see the tmp files in the tmp directory.
I'd checked and tried several chmod permissions (775/777), chown to apache:apache, my own, root, all with no success.
In admin/settings/file-system, I'd tried /tmp, /var/www/tmp and files/tmp or even any other places I can think of. And if the directory doesn't exist, it will auto create by itself and place the .htaccess file inside.
The only difference between my old server and these 2 servers are, the first one is running on CPanel. The latest one doesn't have any control panel at all.
And any Drupal users include myself as admin will receive this error message even the node and files are created successfully:
The selected file /var/www/html could not be copied.
I hope someone could help me on this. Thanks in advanced.

I had something similar when i was migrating Drupal installations from one VM to another. It seemed to me like yourself that the permissions were correct, and the ownership was as well.
* Is the file in use and Apache has control?
* Do you have the correct permissions to access that file(s)?
* Have you re-created your files directory?

I realised the culprit is actually filefield_paths.module with Active updating enabled.

i've been looking for the solution, but it seems no exact science what cause and how to troubleshoot this issue... thanks for sharing the idea and heads up when using filefield (path)... :)

It has to do with php safe mode. Disable safe mode in your php.ini
