I use all the latest versions of Simple News modules. I can't seem to get the 'Name' and 'E-mail address' fields in the simple news block (like the picture on the SRB project page). What am I doing wrong?

Figured it out (rtfm).

Quote form the project page:
"The Simplenews Register module is enabled and the newsletter is set so the registering user must opt out."
