You can't update the radio name field, which means that it won't work on a post as the field gets posted as edit[field_name] so it gets url encoded.
e.g.

....
'#name' -> 'change_it_to_this'

doesn't work

Forms and survey modules are no longer being maintained, use Webform module instead.
