Add support for disabling the list of options

I've applied this patch which allows the list of options to become "locked" by disabling the input fields. It still allows a default value to be chosen, but could be useful when dealing with a pre-defined list of options that cannot be changed. It's utilized in Webform as part of #406486: Allow pluggable select list values.
