I list the Menus, find the category that I want to add a menu item in and click 'Add Item'.
The 'Add Menu Item' tab opens and I enter the Menu Item title, description & link, then click submit and I get a 'Page not found'.
Additionally if I open the 'Add Menu Item' tab and just hit refresh I get a 'page not found' error.

Any suggestions? Sound some kind of cacheing issue or something as it is there the first time, but not when it's refreshed or accessed a second time. I could be off my rocker though.

Additionally I should mention that my first troubleshooting step was to update to the latest version as his website version had not been updated in likely about a year.

I should also mention that I have added next to no modules to this site and it is basically operating on Drupal Core. With the exception of CCK, Admin Menu & Google Analytics.

Considering the lack of activity on this issue and that Drupal v5 is no longer supported by fixes or patches, I am going to close this ticket. If this issue still exists and you want to continue to ask for technical support, please reopen and update this ticket.
