Issues for installations using database prefixes

Hi,

Very nice module - awesome. I wanted to just post a quick FYI that there are a few errors that come up for installations that are using a database prefix. I think there are just a few places from what I could see (around line 91 and 813 - ish) that I think just needs to have the curly brackets added so that Drupal can add the DB prefix.
