I am trying to set custom tags for the blog path on my site.

Name: Blog
Path: blog
Enabled: true

I then enter values in the description and keywords fields and click Save. The form saves with no errors, however when I edit the item again, there are no values in the Description or Keywords field.

An item has been created in the nodewords_custom table

+1 I'm getting the same error. This is killer as it's impossible to go back and see which setting is producing which keywords.

Having the same issue. Not sure if it's part of the problem, but I recently also installed nodewords-nodetype (http://drupal.org/project/nodewords_nodetype)

Nodewords "other pages" to capture dynamically generated pages, and nodewords-nodetype to make new nodes default to certain tags.
