publish / unpublish actions thrown by comment module on publishing moderated comments arent recognized by the trigger module.
attached patch for a quick fix..

the trigger user-interface fix isnt included in the above patch file.
so this patch will only work if you add your trigger actions directly in the trigger_assignments database table with op "publish", "unpublish".
