Generalize the link creation code

There's currently a lot of duplicate between xmlsitemap_node_create_link(), xmlsitemap_menu_create_link(), etc. With the available entity information, we should be able to call a 'xmlsitemap_link_create' function which does most of the work, and then maybe just lets other modules alter what they need to.
