This patch allows simplenews newsletters to have HTML headers and footers. These are configureable on a newsletter by newsletter basis.

In my experience with html email I expect the footer to come before the "Click here to unsubscribe from this newsletter" statement. I see that as the expected place for that type of opt out link in an email. Otherwise, there is the potential for it to get lost in whatever html is placed in the footer and confused with content.
