default to just one subscription mode enabled

Splitting off from #799688: Comment notify settings not being saved..

The module should default to just one subscription mode enabled. This is the best practice for UI and should therefore be default.

I think it should default to all comments (the original purpose of the module) but am willing to be swayed. Anyone have input on which way to default?
