First pass at making the module usable.

- Fixed install / uninstall.
- Moved field & campaign creation from hook_enable() to hook_install().
- Added proper uninstall code that deletes the created fields.
- Moved entity classes to own files to avoid uninstall errors.
- Fixed a warning caused by incorrect usage of _affiliate_ng_register_click().
- Re-exported the default campaigns view.
- Updated for latest Entity API changes.
- Made the Field UI on Campaigns visible in settings.
- Added basic usage information to the README.
- Removed empty folders.
