(Patch) prevent transcoded flv filename collisions

To reproduce this bug, upload two different videos which have the same filename, or differing only in extension (I have done this in cck video field but it is presumably triggered in normal video attachments). When they are transcoded to flv the converted flv filenames will collide and one won't get correctly transcoded.

Attached patch should fix this.
