After activating this module, editing a node, and adding a menu item in the Menu Settings tab, the parent item list is empty. See my attached screenshot. Note that I have many modules added to this site including menu_admin_per_menu. I do not have time to test out all possible conflicting modules at this time. Any help you can provide is appreciated - this module is a great idea!

I thought this was caused by #838186: The module's administration page does not load but now that I look at it, it wouldn't explain why you would have this issue. Taking a look now...

To fix this issue you have to check some menus in module admin page (this worked for me)

Whoops, I see the problem. Uploading a new version. In the meantime, yeah, explicitly check the menus you want in the admin page. The update coming soon will correctly assume the default value to be all menus.
