Sorry - I know I should create a patch for this but for various technical reasons I can't right now so I'm just going to submit this as a suggestion.

In the README

-- First, the multiple_node_access patch allows you to configure the
Domain Access module to use AND logic instead of OR logic when

Should be changed to

I no longer recommend the use of DA Advanced, since it can cause other issues since its development hasn't kept up with API changes.

In fact, I recall removing lines to that effect at one point.
