User targeting and user profiles

This patche solves 2 issues:

- not every publication wants user data targeting (gender, zip, age)
Added a new checkbox to the global settings form for the apt_plugin to disable/enable the User Profile targeting

- added conditions to the user_age, user_zip check, at the moment it would always set the age to 60-64.

In addition, I think there has to be a documentation somewhere for the required profile fields (profile_zip, profile_dob, profile_gender).
