This might be a bug report. I have seen this problem once before.
I have it set up so that new nodes and comments get 2 points each. Reads, recommends (votes), prints, and emails get one point each.
As you can see, this post is old enough to not get the new node juice. It only has 4 reads today (node_counter table) and only 4 comments. Even if all the comments came today (unlikely), how could this node have gotten so much energy? It had none yesterday. The decay interval is 10,800.

If it matters, its nid is 4398. The node pre-dates the installation of Radioactivity.

Could you please give some info on your configuration, at least your db backend and whether you are using memcached acceleration or not.

This is very infrequent and I have no other details about why.
