From the UI, it is not clear whether the php snippet to be entered in the Breadcrumb Visibility text area should include php tags (line 293 of custom_breadcrumbs.admin.inc).

I agree and I will update the field description. Note that this need was also recently pointed out in #871564: Visiblity by Role. I'll leave this open until I update the field description.
