missing file or filename than using filefield paths with domain access module

i don't know what wrong, but with this module i can't add more than one image per node save.

flle path like [type]/image
filename by default

i add 3 images, click save node and i see message, what files can't be uploaded, because they are broken.

on my another project this module works fine, but with domain access i see this issue and i think it's bug.
