update to 5.0

attached patch updates the module to 5.0

also, i threw in another small change which i believe helps usability--the select box on the settings page has a default size of 1, and when you have more than 50 items in a multiselect, that size is a bit maddening... :) the new code makes the size of the select box big enough so that all db tables can be seen at once.

i'll attach the new .info file as well...
