Automatically add on insert not update

Hi ezra-g, I'm wondering if the following patch makes sense to anyone else.

We're using smartqueue_og with the auto-add feature, but we've patched the module so that only on insert (not update) are nodes added to the group's queue.

This ticket is to get other opinions on whether this tweak works better for people.
