Multiple emails being sent to the same recipient and from not appearing.

First of thanks for a great module. It perfectly fills the needs for one of our clients.

We have had an odd issues though (possibly bug?). When filling out the to and from on the send node form.

1. recipient receives two copies of the same email.
2. recipient from address is that of the default site email address rather then from address entered by user
3. Is there a way to send a confirmation address to the user sending the email? ie your email forwarding xyz node has been sent...?
