Bounty for Taxonomy Autocomplete widget

I need a very simple autocomplete form to replace my current (overgrown) taxonomy treemenu for changing location here.

The autocomplete widget will match a single entry against a single vocabulary and return matching terms with their parent term to disambiguate cities & counties with the same name or cities with common names.

Example:
should return Oakland, Alameda County* and Oakland County*, Michigan - to differentiate the city of Oakland in California from the county of Oakland in Michigan
should return Springfield, Sangamon County* plus Springfield, Hampden County* plus...
* bonus points for adding "County" to terms or parents from the county level of the vocabulary hierarchy - this is highly desired, but not critical

Selecting the term (prefer select without clicking [Apply]) will redirect to a specific path structure - /lmv/

I'm hoping to find someone who has implemented this common functionality before and will offer $150 to simply provide the PHP/javascript to embed in a block or $200 for a D6 module that exposes a block. I would like to see live examples of previous implementations and I may offer a bigger bounty if I'm really impressed with what I see.

I can provide specific details of the taxonomy structure (it's generated by the gmaps tools module and integrate either code or module myself. I have almost finished my own implementation, but just can't get the form functioning properly and there are other pressing matters that need my attention. This is a commercial site, but only in the loosest sense - we only exist to help promote live, local music, musicians, and venues. Anyone contributing to this community-building site will also get good karma :-)
