Since this behavior has been reported on Taxonomy in the past, it seems like a Taxonomy issue - but I do notice that the problem first appeared for me when I upgraded Category from the Aug 27 version to the Oct 1 version, and when I back off to Aug 27, the problem goes away. At the least, it might help track down some of the persistent issues others have reported with duplicate entries.
