Form style and "Identify section" integration

Hello everyone.

Due to our wish of time saving and lack of time, we propose the following job:

Integration STYLES and the "Identifiez Vous" section of http://crossmedia-interactive.fr/transport/artregietransport/formulaires... On our "on working" drupal website

Details:
-There are a total of 5 objects to style.
-"Ajouter un objet" (Add an object) is a conditional field, radio buttons (probably nice javascript solution to style like index.html is?)
-Final look must be like as the index.html page does.
-All CCK fields + Conditional Fields are well setup and tested.
-hook_form_alter() in same time must be used to add the "Identifiez vous" section (login/register)

This form has to be done before 13 September at 08:00 GMT.
