Issue Summary
You must make a theme called "manage" or else edit the call to the managed-theme or else nothing will happen for you!
This should be noted either inside a readme or else on the admin/settings/manage page so that someone new to Drupal can figure this out.
Other than that it great though. Much better than hacking at the template php file I think.

Also need to enable the "manage" theme from your admin/theme/settings page so that the drupal theme block can use the theme. This theme can only be used when enabled from admin/theme/settings.

Strange settings. Why we can't choose any themes for "manage" module?
