Say I have a rotor item and I wish to change out the image. I upload my new image and nothing happens. The thumbnail doesn't not update nor does the main image update.

In order to 'update' an image, I have to completely delete my rotor item and recreate it with the new image.

I checked and this isn't a browser cache issue (cleared that out). I cleared out the drupal cache multiple times so that wasn't the issue. This is being recreated on multiple browsers and on multiple computers.

Really not sure... Rotor does no caching of its own. Its either browser caching, page caching, block caching or Views caching.
