The source references 'sites/all/modules/notifications_extra/notifications_nice/button_subscriptions.gif' which does not appear to be included in the package.

Confirmed for Notifications 6.x-2.2. I installed this version and got 3 beta components:

Custom subscriptions 6.x-2.0-beta2
FeedAPI Subscriptions 6.x-2.0-beta2
Nice links 6.x-2.0-beta2

I made my own button, named it button_subscriptions.gif and put it in the folder. works perfect.
