AHAH Helper not running

Issue Summary

We had the helper working on one form and it's been working great. Now it's no longer working and basically when you change the drop down in the ahah demo page it runs forever (same action that happens on our custom module). I'm sure something has changed but I can't find anything and I disabled all our other extra modules and still no avail. It seems to be related to form caching but I don't know enough to even know where to begin looking.

Here are the errors received on the ahah_helper demo page called when you change the drop down:

Anyone have any ideas? I need to find some way to fix (going to see if I can get paid help for it if nothing comes of this).
